Harmonia PT is a cash based physical therapy provider.  If you would like to know if your insurance covers out-of-network services, it is your responsibility to reach out to your insurance company. Check out the insurance guide for assistance here.  If you are submitting to insurance, I will provide a superbill for each visit to be submitted to your insurance for reimbursement.

A credit card is required to hold appointments. Payment for full visit is due at time of service.  Payment is accepted via cash, Venmo, Zelle or credit card. Health Savings Account debit cards are also accepted.

Please note, I have a 24-hour cancellation policy.  Any cancellations within 24 hours may be subject to $150 late cancel fee.

In-person visit at The Nesting Place Located in Yaphank, NY

In-person initial visit (50 minutes) $200

In-person follow up visit (50 minutes) $185
