Harmonia PT is a cash based physical therapy provider. If you would like to know if your insurance covers out-of-network services, please click the blue button “check your insurance” found of this page. If you need further assistance, check out the insurance guide for specific questions to ask when calling your insurance company here. If you are submitting to insurance for out-of-network benefits, I will provide a superbill for each visit to be submitted to your insurance for reimbursement through Reimbusify. Please note, you as the patient are responsible for verify your insurance benefits and obtaining any physician referrals or authorization your insurance company may require for reimbursement. You as the patient are responsible for submitting your superbills for reimbursement, and following up with any issues with reimbursement. Harmonia PT is not responsible for visits not being reimbursed.
A credit card is required to hold appointments. Payment in full for visit is due at time of service. Payment is accepted via cash, Zelle or credit card. Health Savings Account debit cards are also accepted.
Harmonia PT has a 24-hour cancellation policy. Any cancellations within 24 hours may be subject to a $100 late cancel fee. No-show appointments or appointments canceled within one hour of your appointment will be charged a $150 no show fee.
